SPONSOR:  The Cystex® Berry Healthy Bladder Spring Sweepstakes (“Giveaway”) is sponsored by DSE Healthcare Solutions, LLC (“Sponsor”), distributor of Cystex® products.

This Giveaway is in no way sponsored, endorsed, administered by, or associated with, Facebook, NutriBullet™ or Walmart®.  You understand that you are providing your information to Sponsor and not to Facebook, NutriBullet™ or Walmart® when entering and when contacted for prize fulfillment.  Information will not be shared or sold.

ELIGIBILITY:  The Giveaway is open to legal residents of the 50 United States and the District of Columbia, except Rhode Island, who are age 18 or older at the time of entry.  Void where prohibited by law. Employees of the Sponsor, its parent company, subsidiaries, affiliated companies, advertising and promotion agencies, electronic media firm, Robin Leedy & Associates, Inc. (“Giveaway Administrator”), the immediate families of such employees (employees’ parents, siblings, children and spouses) and members of the same household of such employees, and anyone involved in the production, development or handling of this Giveaway are not eligible.  All federal, state, and local laws and regulations apply.  By entering, you verify that you are 18 years of age or older, an eligible U.S. resident and you are giving the Sponsor permission to contact you should you be randomly selected.

HOW TO ENTER:  Submit entry as instructed per the entry form via the Official Entry Form Application (“Giveaway Website”).  Giveaway, including entry period and prizing period begins at 12:00 p.m. E.S.T on April 11, 2014, and ends at 12:00 p.m. E.S.T on May 2, 2014.  Three (3) entrants will be randomly selected in total. One (1) entrant will be randomly selected with announcement around 12:30pm E.S.T. on:

•	4/18/2014

•	4/25/2014

•	5/2/2014

Limit of one entry per person, household or email address.  Tampering and/or multiple registrations from the same entrant may result in ineligibility to receive a prize. All entry periods are in Eastern Time unless otherwise noted. Sponsor reserves the right to delete responses that are considered spam and/or are considered abusive, defamatory or obscene.  Administrator’s computer is the official time-keeping device for Giveaway.  Sponsor is not responsible for incomplete or incorrect mailing addresses or invalid entries.  All entry information collected becomes the sole property of the Sponsor and will not be shared or sold. 

PRIZES:  Total prize value of all three (3) prize packages is approximately $539.79 USD plus shipping and handling depending on winner location in the United States.  One winner to receive one prize package per each entry period; three winners in total.  

Each prize package includes:
•	One (1) bottle of Cystex® Cranberry Urinary Health Complex (ARV $9.99)
•	One (1) NutriBullet™ juice extractor (ARV $119.94)
•	One (1) $50 Walmart® gift card (ARV $50.00)

Any expenses not mentioned above are the winner’s responsibility. If applicable, taxes are the winner’s responsibility.  Prizes are not transferable or redeemable for cash.  No substitution for prize except as may be necessary due to unavailability, in which case a prize of equal or greater value will be awarded at the Sponsor’s sole discretion.  Odds of winning depend upon the number of entries received.  Prize products may vary and may be substituted at the Administrator’s discretion as long as the value is the same.  Limit of one prize per household.

WINNERS:  Winners will be notified via email to the address provided on the official entry form, by announcement on the Cystex®  Facebook page (www.facebook.com/cystex) and by having the prize mailed to the address (no PO boxes accepted) supplied on the Giveaway entry form.  No other notification will be provided.  If this address is in an area where this Giveaway was void by law or outside of the United States, this winner will be considered ineligible and a new winner will be randomly selected for that Giveaway prize.  Winners can expect to receive their prize within four (4) weeks following the Giveaway end date of 5/2/14 or sooner at Sponsor’s discretion.  Winners are required to respond to email notification for prizing confirmation.  If Sponsor is unable to contact a potential winner within five (5) business days, or in the event that a prize notification or prize is returned undeliverable, the prize may be forfeited and, at the Sponsor’s sole discretion, prize may be awarded to an alternate Winner selected in a random drawing from all remaining eligible entries received.  

Winners will be randomly determined by the Giveaway Administrator, Robin Leedy & Associates, Inc., whose decisions regarding this Giveaway are final.  Except where prohibited by law, acceptance of a prize shall constitute and signify the winner’s consent that the Sponsor may use the winner’s name, address (city and state only), likeness and any statements made by the winner about the Giveaway and Sponsor for promotional purposes, in all forms of media, including online on Facebook, Twitter, etc., in perpetuity and without monetary payment or additional consideration.

GENERAL TERMS:  Entrants agree to be bound by these Official Rules.  Participants in this Giveaway, hereby waive, release, and agree to hold harmless Facebook, NutriBullet™ or Walmart® and the Giveaway Sponsor, its parent company, its subsidiaries, affiliates, advertising and promotion agencies, Robin Leedy & Associates, Inc., and all of their respective officers, directors, employees, representatives and agents, from and against, any and all rights, claims and causes of action whatsoever that they may have, or which may arise, against any of them for any liability for any matter, cause of thing whatsoever, including but not limited to any injury, loss, damage, whether direct, compensatory, incidental or consequential, to person, including death and property, arising in whole or in part, directly or indirectly, from their acceptance, possession, use or misuse of, or inability to use a prize in the Giveaway, or their participation in the Giveaway, or their participation in any Giveaway or prize related activity.  Participants acknowledge that the Sponsor and other released entities and individuals have neither made, nor are in any manner responsible or liable for any warranty, representation, or guarantee, expressed or implied, in fact or in law, relative to the prize, including but not limited to its quality, mechanical condition, or fitness, except for any standard limited warranty offered by any manufacturer of the non-cash prize.

INTERNET:  Sponsor and its agents are not responsible for lost, late, misdirected, damaged, incomplete or illegible entries or Internet technical, hardware, software, telephone, or transmission failures of any kind, which may limit a person’s ability to enter the Giveaway.  Sponsor and its agents are not responsible for any injury or damage to entrants’ or any other person’s computer related to or resulting from entering or downloading any materials in the Giveaway.  Sponsor reserves the right to cancel or suspend advertising the Giveaway on the Internet should a computer virus, unauthorized intervention or other cause corrupt the integrity or proper conduct of the Giveaway.  Sponsor reserves the right to disqualify any entrant found to be disrupting the operation of the Giveaway or Giveaway Website.  If the integrity of the Internet portion of the Giveaway is compromised in any manner, Sponsor reserves the right to modify Giveaway and randomly select winners from valid entries received that are unaffected by the problem.  In the event of a dispute as to the identity of any online entrant, entrant will be deemed the individual named on the online entry.  All issues and questions concerning these Official Rules, or the rights and obligations of the entrant and Sponsor in connection with the Giveaway, shall be governed by, and construed in accordance with, the laws of New Jersey, without giving effect to any choice of law or conflict of law rules (whether of New Jersey or any other jurisdiction), which would cause the application of the laws of any jurisdiction other than New Jersey.  WARNING: ANY ATTEMPT BY A PARTICIPANT TO DELIBERATELY DAMAGE THE SPONSOR OR SPONSOR’S WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE GIVEAWAY MAY BE IN VIOLATION OF CRIMINAL AND CIVIL LAWS, AND SHOULD SUCH AN ATTEMPT BE MADE, THE SPONSOR RESERVES THE RIGHT TO SEEK REMEDIES AND DAMAGES (INCLUDING ATTORNEYS’ FEES) FROM ANY SUCH PARTICIPANT TO THE FULLEST EXTENT OF THE LAW, INCLUDING CRIMINAL PROSECUTION.

APPLICABLE LAWS AND JURISDICTION:  With respect to any dispute regarding the Giveaway, your rights and obligations and actions shall be governed by the laws of New Jersey, as if the rules were a contract wholly entered into and wholly performed within New Jersey.  Any dispute relating in any way to your participation in the Giveaway shall be submitted to confidential arbitration in New Jersey.  Arbitration under this agreement shall be conducted under the rules then prevailing of the American Arbitration Association.  The arbitrator's award shall be binding and may be entered as a judgment in any court of competent jurisdiction.  To the fullest extent permitted by applicable law, no arbitration under this Agreement shall be joined to or consolidated with an arbitration involving any other party subject to this Agreement, whether through class arbitration proceedings or otherwise.
