How to Enter: To enter, submit your name, address and email during the Sweepstakes Period December 17, 2012 to January 7, 2012 at 11:59 a.m. Only one entry form per person will be accepted. No mail-in entries will be accepted.
 
Eligibility: The sweepstakes is open to Park Nicollet Facebook fans who are also 13 years of age or older. Entrants must provide a valid email address and must be able to receive a notification from Park Nicollet. This offer is void where prohibited or restricted by law and subject to Federal, State and local laws. Entrants accept the conditions stated in the rules and agree to be bound by the decisions of Park Nicollet.
 
Eligible Entries: Park Nicollet will determine in its sole discretion which entries are valid entries according to these Official Rules. All decisions regarding the selection of the winner are final.
 
Selection of Winner: All qualified entries received during the official entry period will be combined into a single entry pool. The winning entrant(s) will be selected at random by a third-party Facebook application (subject to eligibility verification and compliance with these rules). Entries received before or after the official entry period will be disqualified. The odds of winning a prize depend on the number of eligible entries received during the Promotion Period and the number of votes you receive.
 
Notification of winner: The winner(s) will be notified by a Prize Notification sent to the email address on the entry form. If any prize notice is undeliverable for any reason, or if the winner does not respond within 24 hours, or does not comply with these Official Terms and Conditions, the prize will be forfeited, and another winner will be selected at random from the remaining entries. Unclaimed or unredeemed prizes will not be awarded.
 
Confirmation of Winner(s): The prize(s) will be issued when Park Nicollet has determined, in its sole discretion, the Winner submitted an eligible entry and otherwise complied with the Official Rules. All decisions of Park Nicollet will be final.
